Record of Employment (ROE) Request Form

A Record of Employment (ROE) is an official document issued by an employer when a worker experiences an interruption of earnings.

To be eligible for an ROE from The Job Shoppe, a candidate must experience an interruption of earnings - either by not receiving insurable earnings for seven (7) consecutive calendar days or by having their weekly earnings reduced to less than 60% of their regular weekly pay due to one of the following reasons: layoff or termination, resignation, illness or injury, maternity, parental, or caregiving leave, or an unpaid leave of absence.

If you meet this criteria, please fill out the form below. Requests received by Tuesdays at 2:00 PM each week will be processed on Fridays.
